Kad mēs runājam par erektilās disfunkcijas traucējumiem?

For a long time considered a shameful disease that was difficult to talk about with one’s doctor, erectile dysfunction or male impotence has become easier to talk about since the marketing of specific treatments. These drugs, whose effectiveness has changed the lives of tens of thousands of people, must however be used with caution.

Many men of all ages sometimes experience “breakdowns” due to fatigue, stress, various worries or problems with their partner. These occasional “blackouts” are not erectile dysfunction in the strict sense of the word. In order for erectile dysfunction to be a real problem, the erections must be systematically unstable (or absent) during sexual intercourse and the problems must last for more than 3 months.

What are the symptoms of erectile dysfunction?

Erectile dysfunction, also usually called impotence, is a persistent or recurrent inability to achieve or maintain an erection that allows sexual intercourse. Erectile dysfunction can cause considerable impairment of quality of life. It can occur at any age, but increases with age. The main risk factors are age, diabetes, high blood pressure, high cholesterol and depression.

What causes erectile dysfunction?

Erectile dysfunction can have several causes.

  • Psychological causes: Depression, relationship problems or a poor self-image can be enough to cause erectile dysfunction, as well as the fear of reduced sexual performance. Talking to your partner, your doctor, a psychotherapist or a sexologist can help to unblock the situation.
    Sometimes men who have suffered a heart attack fear that sexual arousal will cause another attack. This fear may manifest itself as impotence. If you maintain a level of physical activity that is compatible with your heart condition, sexual activity is not an additional risk. If you experience chest pain during sex, talk to your doctor.
  • Prostate surgery: Some patients who have had prostate surgery (prostatectomy) may experience erectile problems. Surgery is improving and these cases have become less common.
  • Another disease: Conditions such as high blood pressure, diabetes or atherosclerosis are sometimes accompanied by erectile dysfunction. Diabetes is the leading cause of impotence in men over 50. Hormonal disorders (such as a drop in testosterone levels) are sometimes involved, as well as blood vessel problems.
  • Smoking and alcohol: Smoking reduces sexual ability and can be a factor in aggravating erectile dysfunction, as can alcohol abuse.
  • Medications: A wide range of medications can interfere with the ability to achieve an erection or orgasm. Among the drugs most often involved are those prescribed for high blood pressure, cholesterol or heart problems, for depression, anxiety or Parkinson’s disease, for prostate disorders, but also certain diuretics. Never stop your treatments on your own initiative in the hope of regaining strength! If erectile dysfunction appears a few days or weeks after starting a new treatment, ask your doctor for advice. He or she may be able to change the dosage, prescribe another drug instead, give you lifestyle advice to improve the situation or sometimes prescribe an erectile dysfunction treatment to compensate for the side effect.

What is the diagnosis of erectile dysfunction?

It is advisable to consult a doctor when erectile dysfunction has lasted for more than 3 months or causes physical and/or psychological suffering.

During the consultation, the patient is asked about the characteristics of the erectile dysfunction: sudden or progressive onset, permanent or intermittent erectile difficulties, situations that favour the disorder, etc.

Several medical examinations are systematically carried out in search of a pathology or an underlying cause:

  • An examination of the genitals including a rectal examination to check the prostate;
  • A cardiovascular examination to look for possible high blood pressure or a cardiac disorder that could explain the disorder;
  • A neurological examination to rule out certain pathologies such as Parkinson’s disease, for example;
  • A blood test to check for diabetes, kidney problems or high cholesterol;
  • A psychology or sexology consultation, radiological examinations, etc.

What treatment?

When erectile dysfunction persists despite the implementation of measures to enable a healthy lifestyle and the management of a possible underlying pathology, a drug treatment can be prescribed: sildenafil, tadalafil, vardenafil, etc. These drugs increase the erectile capacity of the penis and the blood vessels.

These drugs increase the blood flow to the penis during sexual stimulation. The erection is more easily achieved and lasts longer. To be effective, the treatment must be taken at least 30 minutes before sexual intercourse. It is advisable not to take more than one dose per day, and not to use it too regularly.

Other treatments exist:

  • The vacuum pump: the penis is placed in the tube connected to the pump generating a suction which generates a blood flow in the penis. The erection is maintained by inserting a constriction ring at the base of the penis to prevent the blood flowing back. The device should not be in place for more than 30 minutes;
  • Eroxel: Eroxel is a safe and legal product available over the counter (without a prescription) which ingredients are extensively tested. There are many positive reviews from users who testify that this product has helped them feel more comfortable with the size of their erect penis. Eroxel Original
  • The penile prosthesis: the penile prosthesis or penile implant is a definitive treatment offered when other treatments are ineffective or contraindicated. The implant is installed, under general or local anaesthesia, in the erectile body of the penis. The device is invisible to the eye. There are semi-rigid implants that are manually raised or lowered by the patient. There are also inflatable implants that are installed in the bursa by means of a pump.
